Two Ways to Make an Appointment

Kernan Hospital Online Appointment Request:

Choose one of the three options listed below that best describes you to get the specific appointment request form you need. Once you fill out and submit the proper form, a Kernan Hospital representative will contact you during the next business day between the hours of 7 a.m. and 7 p.m. to schedule the appointment.

Patients:
You are a patient who wants to make an appointment for yourself.

Note: Pain Management patients must be referred by a physician. This may be the patient’s primary care physician or any specialist who is currently treating the patient.

Friends or Relatives:
You want to make an appointment for someone else (friend or loved one).

Physicians:
You are a doctor who wants to refer a patient.
