The Comprehensive Medical Rehabiltation Program

The Comprehensive Medical Rehabilitation Program at Kernan Hospital provides daily medical management and comprehensive treatment for patients requiring rehabilitation following a variety of surgical procedures or medical treatments. Patients receive an interdisciplinary approach to care from our caring and dedicated staff of healthcare professionals.

The Interdisciplinary Team

Within 48 hours of admission, patients are evaluated by a skilled interdisciplinary team. Once this initial assessment is complete, the interdisciplinary team meets to coordinate care, develop an individualized treatment plan, set rehabilitation goals and establish a projected discharge date.

The members of his or her interdisciplinary team will include a physician, nurses, physical therapists, occupational therapists and a case manager. Speech therapists, recreational therapists, respiratory therapists, and clinical dietitians may also be part of the team as indicated by the patient's needs.

An Interdisciplinary Treatment Approach

All patients of the Comprehensive Medical Rehabilitation Program receive an individualized treatment program. While no two patient treatment plans are exactly alike, most plans feature a combination of therapies and activities designed to help patients improve physical mobility, regain daily living skills and prepare for community re-entry. Additional medical specialty consultations are available if needed, such as pain management, podiatry, wound care and ENT.

Evaluating a Patient's Progress and Care

A patient's interdisciplinary team holds weekly meetings to discuss and evaluate the patient's progress and ongoing care.

Family Involvement

Family members and identified caregivers are encouraged to be active participants in the rehabilitation process. They are invited to attend therapy, educational classes and formalized family training sessions in preparation for care of the patient upon discharge.

The Role of the Case Manager

The case manager is the link between the family and the members of the patient's interdisciplinary team. He or she keeps families informed about their loved one's progress and helps them make necessary preparations for discharge. The case manager also serves as the point of contact for insurance issues, working closely with the family and insurer to identify and address any potential problems.
